Heather writhes in pain from her punishment

In this vid there are sections where Heather is squirming as if being stimulated in a particular way, but the dom does not seem to be doing anything to get this response. Is there some sort of other stimulus we are not seeing? Or has she been worked to a point where any movement causes the clothes pin to move, sending pain shooting through her.
